Changing your address

How to do it

1. Click on the amendment form (pdf, 535 kB).
2. Fill in the form directly online, and sign it digitally. Or print it out and fill it in with a pen in block capitals.
3. Send the amendment form to us.
   • You can do this digitally via the contact form. Choose the 'Health insurance and abroad scheme', then select ‘Change of address'. Add the amendment form as an attachment.
