How do I report a change of address?
Moving house may affect your health insurance and that of your family members. This depends on the country where you are going to live. Find out more about this on this page.
Good to know
- Please first report the change of address to your Dutch health insurer. They will inform the CAK and the health insurer in your country of residence.
- Report the move separately to us using the change-of-address form (PDF, 532 kB) (pdf, 535 kB). This way, you can be sure that post from the CAK will be sent to the right address.
What does a change of address mean for my health insurance?
Moving within your country of residence
Are you going to change address within the country where you already live? In that case, report this to the health insurer in your country of residence as well. In some countries, this will result in you having a different local or regional office.
Moving to another country
I am moving to another treaty country
You will receive a new document from your Dutch health insurer. This will allow you to register yourself and your family members with the designated health insurer in your new country of residence.
Your family members will continue to be insured through the CAK. The amount of your healthcare contribution will change. This is because the country-of-residence factor is different for each country. We will send you a message about this. And the care you receive may also be different. The new health insurer can tell you which healthcare costs are and are not reimbursed.
I am moving to a non-treaty country
In that case, you family members will cease to be insured through the CAK. Ask your Dutch health insurer about how to insure your family members.
Moving back to the Netherlands
Do you live in a treaty country and plan to move back to the Netherlands? In that case, your family members will cease to be insured through the CAK. Ask your Dutch health insurer about how to insure your family members.
