How do I report a death?
Has someone close to you died? We are very sorry for your loss. We will gladly help you deal with the health insurance aspects. This page explains what we need from you and what you can expect.
Good to know
- Payment of the healthcare contribution stops with effect from the date of death.
- We need an official confirmation. This can be a message from the local health insurer or a death certificate.

Step-by-step guide
You can report the death in one of two ways:
- You report the death to the health insurer in your deceased family member’s country of residence. They will then send us an official deregistration notification. Within the EU/EEA, this is fairly quick. In Turkey or Morocco, for example, it may take several weeks.
- You send us a copy of the death certificate. Here’s how to do it:
- Go to the online form Report a death.
- Fill out the form and upload the death certificate.
- Click Send.
What can I expect after this?
We will send you a letter about the cessation of the health insurance and the healthcare contribution.
We will send you an adjusted invoice or annual statement. Was the deceased person the only co-insured family member? If so, you will receive a final statement.
My partner has died, and they were the breadwinner. I am co-insured through the CAK. What should I do?
Unfortunately, you will then no longer be co-insured through the CAK. Contact your local health insurance fund/company. They will be able to advise you on your own health insurance. They will also send us an official notification of your partner’s death. You can also report this to us yourself, but we will need a copy of the death certificate. You can also call us on +31 88 711 5551.
Tip
Also notify the Dutch health insurer of the deceased.
