How do I report a change of address?
Tell us about your move either online or by post. Your relocation may have an impact on your health insurance. This depends on the country where you are going to live. Find out more about this on this page.
Reporting directly
- Click on the amendment form (pdf, 535 kB).
- Fill in the form directly online, and sign it digitally. Or print it out and fill it in with a pen in block capitals.
- Send the amendment form to us.
- You can do this digitally via the contact form. Choose ‘Health insurance outside the Netherlands’, then ‘Notify us of a change of address’. Add the amendment form as an attachment.

What does the move mean for my health insurance?
Moving within your country of residence
Are you going to move within the country where you already live? In that case, report this to the health insurer in your country of residence as well. In some countries, this will result in you having a different local or regional office.
Moving to another country
I am moving to another treaty country
In this case, the health insurance in your previous country of residence will end. You will receive a letter from us with a new S1 or 121 document. This will allow you to register with the health insurer in your new country of residence.
You will therefore continue to be insured through the CAK (Central Administration Office). The amount of your healthcare contribution will change. This is because the country-of-residence factor is different for each country. We will inform you about this. And the care may be different. Your new health insurer can tell you which healthcare costs are and are not reimbursed.
I am moving to a non-treaty country
In this case, your health insurance through the CAK will end. You will need to take out health insurance by yourself in your new country of residence.
Emigrating from the Netherlands

Moving back to the Netherlands
Do you live in a treaty country and plan to move back to the Netherlands? In this case, your health insurance through the CAK will end. You will then need to take out insurance in the Netherlands through a Dutch health insurer. If you need more information, you can call the Zorgverzekeringslijn.
