VOZD
A VOZD (voluntary agreement for healthcare cost coverage) is a scheme that is available to those who have to (re)pay healthcare costs because they were uninsured for a certain period of time. This situation can occur if, for example, you were insured in Belgium but it turned out afterwards that you should have been insured in the Netherlands. If a situation such as this has lasted for more than 4 months, you can no longer register retroactively with a Dutch health insurer.
The conditions
- For a certain period of time, you had health insurance in an EU/EEA country, the United Kingdom or Switzerland. In retrospect, this insurance turned out to be wrong.
- You have documentation from the Social Insurance Bank (SVB) (a Wlz statement) stating that you should have been insured in the Netherlands during that period. Do you not yet have a Wlz certificate? Then please complete the consent form now. You can apply for a Wlz statement with the SVB and send it to us later.
- In the Netherlands, you can no longer take out back-dated insurance.
- For that period, you will pay a contribution to the CAK that is approximately equal to the nominal premium payable to a health insurance company.
Good to know
- The reimbursement of healthcare costs is governed by Dutch regulations.
- We will apply for a healthcare benefit over that period on your behalf, if you are entitled to it.
Examples
- You live in Germany and will be employed by a Dutch company located over the border in Venlo from 1 March 2024. You are under the assumption that your German insurance will continue as normal. On 26 August 2024, you find out that this is not the case. You apply for health insurance with Menzis. This can no longer be back-dated, because more than 4 months have passed. You are therefore uninsured for the period from 1 March to 26 August 2024.
- You live in the Netherlands and have been working in Belgium since 1 January 2024. You have registered with a mutual fund in Belgium, while in the Netherlands, you have registered for a health insurance policy with the health insurance company CZ. Your 16‑year‑old son will also be covered by the health insurance from CZ. You do not disclose to CZ until 8 July 2024 that your son has worked at a supermarket in the Netherlands every Saturday since 1 February 2024. CZ retroactively withdraws your son’s health insurance policy and offers him health insurance starting on 8 July. Your son is uninsured for the period from 1 February to 8 July 2024.
- You live in the Netherlands and receive a pension from Germany. You have registered with a German health insurance company, and are registered for a CZ health insurance policy in the Netherlands. In the Netherlands, you can receive the AOW (National Old-Age Pensions Act) pension. As you were unaware of this, you did not submit an application to the Sociale Verzekeringsbank – SVB (Social Insurance Bank). On 1 May 2024, you still need to submit an application to the SVB. On 12 July, the SVB awards you an AOW pension, backdated to 6 May 2023. You pass this on to CZ. CZ withdraws your insurance policy with effect from 6 May 2023. You apply for health insurance with Univé on 1 July 2024. You are uninsured for the period from 6 May 2023 to 1 July 2024.
Applying for a VOZD: How it works
Check whether a VOZD is beneficial to you
First, calculate whether a VOZD is financially advantageous for you. You will find the annual contributions, excluding the healthcare benefit, in the table below. This will enable you to calculate what the approximate contribution is for the period during which you were uninsured.
Voluntary
Is the contribution higher than your healthcare costs for that period? If so, you do not need to register. Concluding this agreement is voluntary.

Registering with the CAK
Do you meet the requirements for eligibility and want to benefit from a VOZD? Then complete the consent form. This form consents gives us permission to request data from various organisations. The form states which organisations these are.

- Gather your supporting documents together:
- healthcare provider accounts (costs incurred in the uninsured period),
- SVB Wlz certificate (if you do not yet have it, send it to us later).
- Send us the completed form with the supporting documents.

Checks
- We ask CZ and/or the foreign health insurer for information about the period during which you were wrongly insured.
- We ask how much we can reimburse in accordance with the Dutch rates.
- We calculate the amount of the contribution that you must pay to the CAK.
- We ask the Dutch Tax Administration whether you would have been entitled to the healthcare benefit during the uninsured period, and if so, how much.
Tip
For as long as you do not have a VOZD, you remain responsible for any and all payments or payment arrangements. If you were insured with CZ, we have agreed with them that they will not take any debt-collection measures for the duration of the VOZD checks.
Outcome
Following receipt of your Wlz statement, the checks may take several months. This depends on how quickly other authorities respond. You will then receive a letter from us with our findings. There are three possibilities:
You are not entitled to a VOZD
You may still be entitled to health insurance for the requested period elsewhere. Or you have no obligations under the Wlz, for example because you did not have any income in the Netherlands.
You are entitled to a VOZD
- You conclude an agreement with us.
- We pay your healthcare costs during the period that you were uninsured. We make this payment directly to the healthcare provider. If you have already paid off the amounts yourself, we will offset that amount with the contribution you have to pay to the CAK. We do not pay out any contributions or excess.
- You pay a contribution to the CAK for the period in which you were uninsured. We will send you an invoice for this.
We advise you not to have a VOZD
Sometimes, it is not beneficial for you to have a VOZD. For example, if your healthcare costs are lower than the contribution you have to pay to the CAK for the period during which you were uninsured.
Example: You are uninsured from 1 March to 1 August 2023. During that period, you incur healthcare costs amounting to EUR 400.00. You are not entitled to the healthcare allowance. You would have to pay a contribution to the CAK of EUR 1 889.00 x 5/12 months = EUR 787.08.
